Wb television - TV AfFILIATES

To be eligible for WB Television - TV Affiliates (tier 2) non-union production benefits, you must be paid by a TV Affiliates Participating Employer and expected to work on average 30 hours/week. You are eligible on the first of the month following 30 days of continuous employment.

Enrollment checklist

• Use the Medical Plan Comparison chart to compare the current Aetna HMO, POS and Basic PPO plans.

• Use the Dental Plan Comparison chart to compare the current Aetna DMO and PPO plans.

• Use Provider Search to find in-network medical providers and dental providers.

• Complete and submit the Benefits Enrollment Form. Drag and drop or save it first in your computer, then upload via the Secure Web Link for Employee Forms.

• Complete and submit the Life Insurance Beneficiary Form and upload to our Secure Web Link for Employee Forms. You will be automatically enrolled in Company-paid basic life and AD&D coverage but you must complete the beneficiary form.

• Complete and submit the Consent for Electronic Delivery of Benefit Communications. Due to current circumstances, we will use email and website postings to deliver benefit communications.
