A. The major provisions of our benefit plans including eligibility are described in the applicable Summary Plan Descriptions. Click on Resources, select Forms & Documents, then Click on the applicable Summary Plan Description.
Q. How do I know if I'm eligible for benefits?
A. Click on New/Rehire for your complete enrollment package. If you want to enroll in health coverage, you must complete and return the Benefits Enrollment form within 30 days of your eligibility date through the secure web link posted. Otherwise, you must wait until the next Open Enrollment period.
Q. How do I enroll in medical, dental & vision coverage?
A. Use Provider Search on Aetna's website and follow the prompts. You can change your election by contacting Aetna directly. You don't need to wait for Open Enrollment.
Q. If I select the Aetna medical HMO or dental DMO, how can I find a Primary Care Physician or Dentist and the office ID number needed for the Enrollment form?
A. In most cases, it's the first of the month following 30 days of continuous employment. Refer to the Summary Plan Description for complete details.
Q. When does my health coverage begin?
A. Generally 7-10 business days. You can also print them out yourself on Aetna's website.
Q. When will I get my ID cards?
A. It’s the time of year when you can make changes to your health coverage such as switching plans or adding/deleting dependents. It starts in late June or early July with changes effective August 1st. You'll be notified via email and information will be posted.
Q. What is Open Enrollment?
A. Click on Life Events.
Q. What if I get married, divorced, have a baby, or lose/gain other coverage?
A. It is very important that we have your current address on file. Make sure to notify us either by email or submit secure the Benefits Change Form.

Q. How do I get a copy of my W2?
A. Call Entertainment Partners at (818) 955-6022 or email them at 1095inquiry@ep.com. Form 1095-C is for your records only and does not need to be appended to your tax returns.
Q. How do I get a copy of Form 1095-C?
A. Visit www.theworknumber.com (available 24/7) and select “I’m a Verifier”, use Employer Code #13481 (for Cast & Crew / BTL), then input employee’s social security number.
