MEDICARE PART D NOTICE OF CREDITABLE COVERAGE
The purpose of this notice is to advise you that the prescription drug coverage under the Group Benefits Plan is expected to pay out, on average, at least as much as the standard Medicare prescription drug coverage will pay in 2024/2025. This is known as "creditable coverage".
HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT (HIPAA) NOTICE OF PRIVACY PRACTICES
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) imposes numerous requirements on the use and disclosure of individual health information by the Company health plans. This information, known as protected health information, includes almost all individually identifiable health information held by a plan - whether received in writing, in an electronic medium, or as an oral communication. This notice describes the privacy practices of these plans: Medical, Dental, Vision & FSA. The plans covered by this notice may share health information with each other to carry out treatment, payment, or health care operations. These plans are collectively referred to as the Plan in this notice, unless specified otherwise.
THE AFFORDABLE CARE ACT (ACA) Form 1095-C
As a part of the ACA, the IRS requires that the Company provide you with a tax form to verify the healthcare coverage available to you, your spouse and/or dependents.
Form 1095-C is for your records only and does not need to be appended to your tax returns. Essentially, you don’t need to wait to receive the form in order to file your taxes. If you have questions regarding your Form 1095-C, please contact Entertainment Partners at (818) 955-6022 / 1095inquiry@ep.com. The form will be sent to the mailing address that the Benefits Department currently has on file. Please call us to make sure that it’s current.
HEALTH INSURANCE MARKETPLACE EXCHANGE NOTICE
Effective 2014, there is a new way to buy health insurance: the Health Insurance Marketplace. To help you evaluate options for you and your family, this notice provides some basic information about the new Marketplace and employment based health coverage offered by the Company.
THE UNIFORMED SERVICES EMPLOYMENT AND REEMPLOYMENT RIGHTS ACT (USERRA)
USERRA protects the job rights of individuals who voluntarily or involuntarily leave employment positions to undertake military service or certain types of service in the National Disaster Medical System. USERRA also prohibits employers from discriminating against past and present members of the uniformed services, and applicants to the uniformed services.
HEALTH INSURANCE PREMIUM PAYMENT (HIPP) PROGRAM - STATE OF CALIFORNIA
The California Department of Health Care Services will pay health insurance premiums for certain persons who are losing employment and have a medical condition that requires a physician’s treatment.

State law (SB 1905 / PA 102-0630) which took effective on August 27, 2021 requires employers to make additional health plan disclosures to Illinois employees.
